I love hand cases! Interesting rare case! Parosteal osteosarcoma of the first metacarpal? NO! Looks like a parosteal osteosarcoma especially with the cleft sign but turns out to be Benign Fibrosseous Psuedotumor of the Digit! The tumor was causing patient significant difficulty with use of the hand and pain. The carpal tunnel was opened and the median nerve dissected into all its branches. The motor branch was protected and then the interval between APB and FPB developed bluntly down to mass and metacarpal. The base of mass exposed and transected and mass removed. It has very low potential for recurrence after removal. The volar arch was also mobilized and protected to preserve blood supply. Interesting case. Patient intact postop. Hand saved!
